Look for a form number and you will not find one: the declaration of non-conviction is a letter you write yourself. The authorities publish a model, which we reproduce word for word: that wording is the one registries recognise. Four statements have to appear in it:
Its full name says it: non-conviction and parentage. You state your parents' names, and for your mother it is her birth name that belongs there, not her married name. It is the most common reason for rejection.
Commercial, artisanal or liberal: the official page on registering a sole trader lists it among the documents concerning the entrepreneur, without distinguishing what you sell. Being a micro-entrepreneur does not exempt you.
You declare on your honour that you have received no criminal conviction and no civil or administrative sanction barring you from trading. Inaccurate information given in bad faith costs a €4,500 fine and six months' imprisonment, under article L. 123-5 of the French commercial code.
